投標人需提供以下文件

*（*）投標人須按招標文件第一冊第四章格式IV-*.資格證明文件格式的要求填寫和提交有關資料。
*（*）投標產(chǎn)品原產(chǎn)地在關境外的投標人必須是制造商；或者是制造商直接授權的代理商（進口產(chǎn)品開標前未進入中國關境內的，投標人必須是在關境外注冊的公司；進口產(chǎn)品開標前已進入中國關境內的，投標人必須是在關境內注冊的公司），代理商投標需具備以下條件：
①必須提供制造商對本次投標的唯一正式授權書（授權書必須由制造商直接授權，不允許二級授權）；授權書中應包括“制造商對產(chǎn)品及售后服務質量負責”的相關內容和制造商聯(lián)系人及聯(lián)系方式，并確保制造商聯(lián)系人能夠協(xié)調處理項目執(zhí)行過程中的重要事項。（對于此要求，制造商也可在授權書外另行出具書面承諾/說明），否則不接受代理投標；
②投標人在當?shù)匾婪ǖ怯涀缘臅r間距開標日滿三年，需提供營業(yè)執(zhí)照等注冊登記的有效證件復印件；
③距開標日三年內，投標人須具有同類產(chǎn)品至少兩個不同年份的銷售業(yè)績，需提供證明其銷售業(yè)績的合同或訂單復印件（至少提供*份不同年度的合同或訂單復印件）或用戶證明，并由投標人代表簽字或蓋章。
*（*）投標產(chǎn)品原產(chǎn)地在關境內的投標人必須是制造商（不接受代理商投標）。
*（*）投標人未被中國石油招標投標網(wǎng)(www.cnpcbidding.com)暫?；蛉∠稑速Y格。（根據(jù)《中國石油天然氣集團有限公司投標人失信行為管理辦法》的規(guī)定，失信積分達到*分時，暫停失信行為人投標資格半年；失信積分達到*分時，暫停失信行為人投標資格一年；失信積分達到**分時，暫停失信行為人投標資格兩年。暫停期滿后恢復投標資格，有效期內的失信記分繼續(xù)執(zhí)行。失信積分超過**分或在暫停期間再次發(fā)生失信行為的，取消投標資格三年。）上述內容以評標委員會現(xiàn)場在對應網(wǎng)站的查詢結果為準。
*（*）投標人未被納入中石油集團公司供應商、承包商、服務商黑名單（集團公司供應商、承包商、服務商黑名單可在中國石油招標投標網(wǎng)/www.cnpcbidding.com查詢）。上述內容以評標委員會現(xiàn)場在對應網(wǎng)站的查詢結果為準。
*（*）距開標之日前一年內，投標人在中國石油物資有限公司組織的招標活動中對評標結果的無效投訴（包括中石油集團公司機電辦處理結果為：不予受理、駁回、維持原評標結果）達到一次及以上的，投標將被否決。
*（*）投標人須提供開戶銀行在開標之日前三個月內開具的資信證明原件或該原件的復印件（由投標人代表簽字或蓋章）。
*（*）第八章中要求的其他合格的資格證書。

① The bidder should provide a letter with official authorized by Manufacturer for the bid；
② the Agent shall get registered in accordance with the local law for at least three years from the bid opening date and copies of valid registration documents such as business license shall be provided;
③ The bidder shall have sales performance for similar products in two different years within three years from the bid opening date. Copies of the contract / purchase order evidencing its sales performance or Certifications from Enduser should be provided and signed or sealed by the Bidder;
(*)The bidder whose product origin is within the territory of the Customs must be the manufacturer (agent bidding is not accepted)
(*) The bidder has not been suspended or cancelled the bidding qualification by petrochina Tendering and Bidding website (www.cnpcbidding.com). (According to the provisions of the Measures for the Administration of Dishonest Behavior of Bidders of China National Petroleum Corporation Limited, the bidder's bidding qualification shall be suspended for half a year if the dishonest score reaches * points; The bidder's bidding qualification shall be suspended for one year if the breach of trust score reaches * points; The bidder's qualification for bidding is suspended for two years after the breach of trust score reaches ** points. If the breach of trust score is **.* points or above, or the breach of trust occurs again during the suspension period, the bidding qualification within the scope of the group company will be cancelled for three years.
